   This booklet will enable you to create a record of vital information as well as your wishes and desires for your final needs.
   Before completing the information in this booklet, talk with your family. Listen to their concerns. 'Toward Peace of Mind' is meant to help your loved ones at a time when they will be enduring heavy burdens. By recording your wishes and desires, along with important information, you will ease some of those burdens.
   We strongly urge you to periodically review the information contained herein and update it as needed. We will be happy to supply additional copies should you need them.
   The booklet should be kept in a safe place where family members will have immediate access. We recommend that you NOT keep it in your safety deposit box. Upon your death your safety deposit box may be sealed until after your funeral. You may wish to instruct your loved ones to deliver this booklet to your funeral director upon your passing.

A Message to My Loved Ones.
I have prepared this booklet in order to relieve you of some of the burdens I know you will face at the time of my death. I have recorded my desires on a variety of items pertaining to my funeral and final arrangements.
By making these decisions in advance, I hope I have lifted some of the burdens off your shoulders, so that you can reflect on the warm memories of the time we had together.
